The Contact Us  at the bottom of the page links to only a form, there is no address nor a phone number. The form's drop-down selection only goes to billing.  My Chart is essential to healthcare management with the way the administration is set up at Cascades East Family Medicine. During my doctor's visit, Dr. Jentzi and I agreed I would email more information. Dr. Jentzi said, "Good! That goes straight to my inbox that I check every evening." Dr. Jentzi name was not listed on the drop-down menu to be able to write her, thus why contacting for tech support was important. I call Cascades East Family Medicine for assistance.  The front desk transfers me to a recording. The recording states to call another number for My Chart help. I am not prepared to write the number.
